It is common to equate severity of angina with risk of fatal cardiac events. There is only a weak relationship between severity of pain and degree of oxygen deprivation in the heart muscle (i.e. there can be severe pain with little or no risk of a heart attack, and a heart attack can occur without pain).

The annual mortality in patients with stable angina with documented coronary artery disease ranges from 1-4%. However, the prognosis varies widely depending on various factors like duration and severity of symptoms, resting ECG abnormalities, abnormal left ventricular function and associated comorbidities[1] Impairment of left ventricular systolic function adversely influences the long term prognosis of patients with chronic stable angina. In patients with three-vessel coronary artery disease, the presence of ejection fraction of less than 50% or clinical evidence of heart failure is associated with almost three times higher mortality than in patients with normal left ventricular function and a similar extent of CAD<ref>Braunwald, Eugene (2003).
