Physical Examination

Physical examination in acute pancreatitis serves three purposes: (1) supporting the clinical diagnosis as one component of the revised Atlanta criteria, (2) assessing hemodynamic status and identifying signs of organ failure that define disease severity, and (3) detecting findings that suggest an alternative diagnosis. Physical findings are nonspecific and have only moderate diagnostic accuracy (positive likelihood ratio 3.2; negative likelihood ratio 0.8), so examination must always be interpreted alongside biochemical markers and, when needed, imaging.[1][2]

General Appearance

Patients with acute pancreatitis typically appear acutely ill, often lying still or leaning forward (the "pancreatic position"), as movement and lying supine exacerbate the pain. Diaphoresis and pallor may indicate hypovolemia or hemodynamic compromise.

Vital Signs

Vital-sign abnormalities in acute pancreatitis reflect the systemic inflammatory response and third-space fluid losses:

  • Tachycardia — common; may reflect pain, hypovolemia, or systemic inflammatory response syndrome (SIRS).
  • Hypotension — suggests significant intravascular volume depletion or early organ failure; systolic blood pressure <90 mmHg that is not responsive to fluid resuscitation corresponds to a cardiovascular score of ≥2 on the modified Marshall scoring system, defining cardiovascular organ failure.[3]
  • Fever — low- to moderate-grade fever is common; high fever (>38.5°C) or fever developing after the first week should raise concern for infected necrosis.
  • Tachypnea — may reflect pain-related splinting, pleural effusion, or early acute respiratory distress syndrome (ARDS) in severe disease.

The presence of ≥2 SIRS criteria (temperature >38°C or <36°C, heart rate >90/min, respiratory rate >20/min or PaCO₂ <32 mmHg, WBC >12,000 or <4,000/mm³) at admission is a strong predictor of organ failure and severe disease and should be assessed as part of the initial evaluation.[4]

Abdominal Examination

  • Epigastric tenderness — the hallmark finding; tenderness is typically localized to the upper abdomen.
  • Abdominal distention — frequently present due to ileus or retroperitoneal inflammation. In severe cases with marked abdominal distention, intra-abdominal hypertension should be considered and may warrant bladder pressure monitoring.[5]
  • Decreased bowel sounds — reflect associated ileus.
  • Guarding — voluntary guarding is common; involuntary rigidity is unusual in uncomplicated acute pancreatitis. The presence of board-like rigidity or rebound tenderness should raise concern for a perforated viscus or other surgical emergency rather than uncomplicated acute pancreatitis.[2]
  • Palpable epigastric mass — may indicate a pancreatic pseudocyst, walled-off necrosis, or phlegmon; more common in late presentations or recurrent disease.

Cutaneous Signs

  • Grey Turner sign — ecchymosis of the flanks, produced by tracking of hemorrhagic retroperitoneal fluid along fascial planes to the subcutaneous tissues via a defect in the flank fascia.
  • Cullen sign — periumbilical ecchymosis, resulting from retroperitoneal blood diffusing along the falciform ligament to the subcutaneous periumbilical tissues.
  • Subcutaneous fat necrosis — tender, erythematous nodules (typically <1 cm), often on the extremities; caused by circulating pancreatic lipase. Rare.

Grey Turner and Cullen signs occur in fewer than 3% of patients with acute pancreatitis and are not specific — they can occur with any cause of retroperitoneal hemorrhage (e.g., ruptured abdominal aortic aneurysm, trauma). Their diagnostic significance is therefore low. When present, they generally indicate necrotizing or hemorrhagic pancreatitis and are associated with more severe disease.[6]

Jaundice

Scleral icterus or jaundice in a patient with acute pancreatitis suggests biliary obstruction (choledocholithiasis) as the underlying etiology or, less commonly, compression of the common bile duct by pancreatic edema or a pseudocyst.

Pulmonary Examination

  • Decreased breath sounds at the lung bases — most commonly due to left-sided or bilateral pleural effusions, which occur in 15–20% of patients with acute pancreatitis.
  • Basilar crackles — may indicate atelectasis or early ARDS.
  • Shallow respirations — result from diaphragmatic irritation by peripancreatic inflammation or pain-related splinting.

The presence of a pleural effusion at admission is included in the BISAP score as a predictor of mortality.

Rare Extrapancreatic Findings

  • Purtscher-like retinopathy — a rare complication caused by complement-mediated leukoembolization of retinal precapillary arterioles. Acute pancreatitis accounts for approximately 12% of reported Purtscher/Purtscher-like retinopathy cases. Patients may report sudden decreased visual acuity or scotomas, typically occurring days after the onset of pancreatitis. Funduscopic findings include cotton-wool spots, retinal hemorrhages, and Purtscher flecken. This finding is not routinely screened for but should prompt urgent ophthalmologic consultation if visual symptoms develop.[7]
  • Tetany or Chvostek/Trousseau signs — may indicate hypocalcemia, which can occur due to saponification of peripancreatic fat by free fatty acids. Hypocalcemia is a marker of disease severity.

Physical Examination Findings Suggesting Alternative Diagnoses

Certain examination findings are atypical for acute pancreatitis and should prompt consideration of alternative or coexistent diagnoses:

  • Board-like rigidity and diffuse rebound — perforated viscus.
  • Pulse deficits or pulsatile abdominal mass — aortic dissection or ruptured abdominal aortic aneurysm.
  • Cervical motion tenderness or adnexal tenderness — pelvic pathology (ectopic pregnancy, PID).
  • Pain in the right lower quadrant — appendicitis.
  • Absent femoral pulses or mottled lower extremities — aortic catastrophe or mesenteric ischemia.

Physical Examination in Severity Assessment

While formal severity grading relies on the modified Marshall score and imaging, certain physical examination findings at presentation correlate with higher risk of severe disease:

  • SIRS criteria met at admission.
  • Hypotension (systolic BP <90 mmHg not responsive to fluid resuscitation).
  • Tachypnea or oxygen desaturation.
  • Altered mental status.
  • Signs of significant third-space losses (marked abdominal distention, peripheral edema).
  • Pleural effusion on examination.

Close serial reassessment of vital signs and abdominal examination during the first 48–72 hours is essential, as initially mild disease can progress to severe acute pancreatitis with organ failure.[4]

Clinically Actionable Recommendations

  • Assess and document SIRS criteria at admission — their presence is among the strongest early clinical predictors of severe disease.
  • Perform serial abdominal and vital-sign reassessments during the first 48–72 hours to detect clinical deterioration.
  • Do not rely on the absence of Cullen or Grey Turner signs to exclude severe or necrotizing pancreatitis — these signs are rare and late.
  • If board-like rigidity, diffuse rebound, or hemodynamic instability without clear cause is present, pursue urgent cross-sectional imaging to exclude a surgical emergency.
  • If a patient reports visual changes during an episode of acute pancreatitis, obtain urgent ophthalmologic consultation to evaluate for Purtscher-like retinopathy.
