Other diagnostic studies for bursitis include aspiration of the bursal fluid. It may be helpful in differentiate septic and aseptic bursitis.

Aspiration of the bursa

Indications for aspiration of bursal fluids include:

• Rolling out septic bursitis
  • Presence of local inflammation
  • Fever
  • Adjacent skin infection
• Rolling out crystal deposite disorders
• History of gout and pseudogout

Type of Bursitis	Aspiration of bursal fluids
Septic bursitis	White blood cell count (WBC) greater than 5000 to 20,000 mm3 with left shifts High protein and lactate level Low glucose level Positive culture and Gram stain
Aseptic bursitis	White blood cell count (WBC) <500 mm3 Normal protein and lactate level Normal or low glucose level Negative culture and Gram stain
Bursitis associated with gout	Positive monosodium urate crystals
Bursitis associated with pseudogout	Positive calcium pyrophosphate crystals positive
Bursitis associated with rheumatoid arthritis	Positive cholesterol crystals
