Overview
Surgical repair has been the procedure of choice for closure of atrial septal defect. It is still preferred for primum, sinus venosus, coronary sinus atrial septal defect.
Steps during surgery
1) Median sternotomy incision.
2) Sternum is split in the midline.
3) Direct arterial and double venous (superior vena cava and inferior vena cava) cannulation are performed.
4) Cardiopulmonary bypass applied
5) Aorta clamped
6) Heart is arrested with a cardioplegia solution.
7) Right atrium is opened.
8) Defect repaired either by continuous prolene suture or with the use of patch
9) Patch can be natural (autologus pericardium), bovine pericardium or artificial polytetrafluoroethylene (PTFE) or Dacron.
