Infective Endocarditis Prophylaxis Regimens [1]

Oral Surgery

The target of the infective endocarditis prophylaxis is Streptococcus viridans:


Able to take Oral Medication
Preferred Regimen
Adult:Amoxicillin 2 gm as a single dose 30-60 min before procedure
Pediatrics:Amoxicillin 50 mg/kg as a single dose 30-60 min before procedure
Unable to take Oral Medication
Preferred Regimen
Adult: Ampicillin 2 g IM or IV as a single dose 30-60 min before procedure
Pediatrics: Ampicillin 50 mg/kg as a single dose 30-60 min before procedure
Alternative Regimen
Adult: Cefazolin or Ceftriaxone 1 g IM or IV as a single dose 30-60 min before procedure
Pedicatrics:Cefazolin or Ceftriaxone 50 mg/kg IM or IV as a single dose 30-60 min before procedure
Age 1—4 Weeks
Preferred Regimen
Ampicillin 200 mg/kg/day IV q6—8h
AND
Cefotaxime 150—200 mg/kg/day IV q6—8h
Alternative Regimen
Ampicillin 200 mg/kg/day IV q6—8h
AND
Gentamicin 2.5 mg/kg IV q8h
OR
Tobramycin 2.5 mg/kg IV q8h
OR
Amikacin 10 mg/kg IV q8h
Age 1—23 Months
