- 1. Chronic enterovirus meningoencephalitis
- Preferred regimen: supportive care ± Pleconaril (investigational)
- 2. HSV-2 meningitis
- Preferred regimen: supportive care AND Acyclovir 10 mg/kg IV q8h for 10–14 days, followed by Valacyclovir 1 g PO tid
- 3. Mollaret's meningitis
- Preferred regimen: supportive care AND Acyclovir 400 mg bid–tid OR Famciclovir 250 mg PO bid OR Valacyclovir 500 mg PO qd
- 4. VZV meningitis
- Preferred regimen: supportive care AND Acyclovir 10 mg/kg IV q8h for 10–14 days, followed by Valacyclovir 1 g PO tid
