HOW SUPPLIED

12.5 mg tablets in bottles of 100 (NDC 60505-0003-6) and 1000 (NDC 60505-0003-9), 25 mg tablets in bottles of 100 (NDC 60505-0004-6) and 1000 (NDC 60505-0004-9), 50 mg tablets in bottles of 100 (NDC 60505-0005-6) and 1000 (NDC 60505-0005-9), and 100 mg tablets in bottles of 100 (NDC 60505-0006-6) and 1000 (NDC 60505-0006-9).
