Introduction to the History and Symptoms Page

  • The page name should be "(Disease name) symptoms", with only the first letter of the title capitalized.
  • Goal: To describe the typical symptoms that a patient verbalizes. Remember this is different from signs which are clinical terms for physicians.
  • As with all microchapter pages linking to the main page, at the top of the edit box put {{CMG}}, your name template, and the microchapter navigation template you created at the beginning.
  • Remember to create links within Wikidoc by placing [[square brackets]] around key words which you want to link to other pages. Make sure you make your links as specific as possible. For example if a sentence contained the phrase anterior spinal artery syndrome, the link should be to anterior spinal artery syndrome not anterior or artery or syndrome.   • Remember to follow the same format and capitalization of letters as outlined in the template below.
  • You should include the name of the disease in the first sentence of every subsection.

Overview

  • The overview section should include the disease name in the first sentence.
  • The goal is to summarize the history and symptoms page in several sentences. This section can be the same as the history and symptoms segment on the overview page.

Template

  • First Sentences:
The hallmark of [disease name] is [finding]. A positive history of [finding 1] and [finding 2] are suggestive of [disease name]. The most common symptoms of [disease name] include [symptom 1], [symptom 2], and [symptom 3].
  • Examples:
Example 1: The hallmark of Ebola virus infection is hemorrhagic fever. A positive history of high-grade fever and recent travel to West Africa are suggestive of Ebola virus infection. The most common symptoms of Ebola virus include fever, sore throat, and bloody diarrhea.
  • Additional Sentences:
Less common symptoms of [disease name] include [symptom 1], [symptom 2], and [symptom 3].
A positive family history of [disease/malignancy] may be present.
  • Examples:
Less common symptoms of colorectal cancer include constipation and abdominal pain.
A positive family history of Retinoblastoma may be present.

History

  • Here you should describe the important components in the patients history that should be given particular focus. Remember to include pertinent points from the past medical history, family history, social history (including occupational, alcohol use with amount and frequency, smoking, drug use) and allergies.
  • Remember this is not the section to describe the natural history of the disease or the historical perspective.

Common Symptoms

  • In this section you can list the common symptoms that the patient may experience.

Less Common Symptoms

  • In this section you can list the less common symptoms that the patient may experience.
  • For an example of the the less common symptoms section within a history and symptoms page, click here.

References

  • References should be cited for the material that you have put on your page. Type in {{reflist|2}}.This will generate your references in small font, in two columns, with links to the original article and abstract.
