- Infections such as Epstein Barr virus (EBV), HTLV-1, HHV8, Hepatitis C virus, Helicobacter pylori
- Environmental factors such as drugs (phenytoin, digoxin, TNF antagonist), pesticides, solvents, organic chemicals, radiation exposure, etc.
- Immunodeficiency states such as severe combined immunodeficiency, Wiskott-Aldrich syndrome, AIDS, and immunosuppressive therapy
- Autoimmune diseases such as rheumatoid arthritis, Sjögren's syndrome, celiac disease, and Hashimoto's thyroiditis
